Notice to Individual - Use of InformationIn compliance with the Citizen Complaint Act of 1997, a "Citizen Complaint" form has been developed and is being provided to you on the internet. This form is being made available to you should you want to register a complaint or provide comments relating to the performance of the Department of Financial Institutions.
This form is not to be used for complaints against a state chartered financial institution regulated by the Department of Financial Institutions. Consumer complaints can be submitted online at http://www.dfi.ca.gov/consumers/consumer_assistance.html.
If you like to register a complaint or comment about the department, you will need to print the Citizen Complaint Form and send the completed form to:
