How to Apply

The Standard State Application Form 678 is available on-line below in two formats for your convenience. Please remember you MUST follow the instructions on the Exam Bulletin indicating where to submit your application for each exam. Simply filling out an on-line application will not submit your application. You must print your application, sign it and submit it to the testing office address listed on the bulletin.

• The Acrobat Application allows you to fill out the application and print it; you cannot save the information for future use.
• The Online Application (secure database) is a database driven Web application that generates an exact replica of the official State Application without actually filling one out.
